Hi so I had my methotrexate 2 weeks on Monday. My levels on Monday gone were at 2000 still but I’ve started to bleed again today. It’s light and brown but the side hurts a little too. Like a cramping. I’m scared it’s a rupture or is it my period or just normal from the shot i don’t even know anymore… anyone and similar experience ?
Dear Annieanxious,
I am so sorry to hear of your ectopic pregnancy loss, It can be a frightening time and you have friends here who understand.
Bleeding and some tummy cramping are common with methotrexate treatment, however as pain is so subjective, if you are concerned or if it is worsening, please seek urgent medical advice.
We have more information on methotrexate treatment on our website here

Hey, it’s a really confusing time and when it was my experience I didn’t feel the nurses explained everything properly at all so I was just left confused.
So my understanding is when there is high levels of hcg in you, your body will not ovulate and therefore you will not menstrate. The bleeding you are experiencing is likely miscarriage bleeding, if you ruptured I think you would know it from the severe pain. I remember the levels of bleeding were quite extreme at times like a tap blood gushing out but I wasn’t in pain at those points.
So what will happen is you will likely continue bleeding, get to around the 20 hcg, then be let go by the hospital, then all the bleeding will stop as your hcg is at 0, then you will likely ovulate and then two weeks later will be your first period.
Hope that helps, if you are concerned definitely talk to a nurse or doctor.

Thank you both the reply. My levels have dropped down to 500. I was going to return to work today but I’m still spotting and cramping. I told the nurse today who said it can be normal too. It’s just such a worry isn’t it!
A drop to 500 is good, you are getting there so that’s positive.
I didnt stop bleeding or stop having pain until my hcg was near 0. So its pretty normal to still have the same symptoms even at 500. Try not not to worry to much. It is going in the right direction.
