I am so sorry to hear of your ectopic pregnancy loss,
It is common to begin bleeding a few days following injection with methotrexate and this bleeding may persist anywhere between a few days to up to six weeks.
Not experiencing vaginal bleeding after being treated with methotrexate is not something you have to be too worried about. Some will not experience bleeding as a side effect with this medical treatment option. You may find you start bleeding as your hCG drops, but don’t worry if you don’t bleed.
To check that the treatment is working, it is important to attend regular appointments with your healthcare professionals to have your hCG levels monitored.

I am currently suffering the same. I would like to ask if it’s okay to ask for an update of how you are at the moment? Have you start bleeding? How are your hcg levels? Do you have any symptoms? Thank you. And praying for our recovery.
I started bleeding at 6 weeks, they said it was a miscarriage, then 8 weeks said it was etopic and had the mtx injection.
What I would say is in my own experience after mtx it didn’t make me bleed more or cramp more in the first week. Methotrexate made me dehydrated, lose my appetite, feel rough and my hcg drop. That’s it.
So I personally wouldnt expect loads of bleeding and cramping, but if you are concerned you should ask the nurse when you visit the hospital again but I feel like as long as your hcg drops that’s all they care about.
