Hello, I have so many questions that I know are difficult to answer at this stage. I had an ectopic 5 months ago on my right side. My body had expelled it on its own into my abdomen requiring surgery to clean out the blood, my doc said my tube looked fine so she left it. I later needed methotrexate for remaining unseen tissue.
Here I am now, pregnant again. I got a very faint positive 8 days after ovulating and it has been getting increasingly darker since. I had an ultrasound today at 3+5, not expecting to see anything. The tech found an old cyst on my left ovary and a more recent cyst on my right side and a small undetermined sac in my endometrium measuring 4 wks. I don’t know my hcg levels yet but I will have follow up draws and another u/s on Friday but I was wondering if there was any information regarding how soon pseudosacs form in ectopic pregnancies? I don’t feel optimistic knowing that I ovulated on the side I had an ectopic previously. If it were potentially a gestational sac why would it measure 2 days ahead? I am 100% sure of my conception/ovulation dates. Like, unless sperm have the ability to travel back in time, there’s no way I’m 4wks.

Im not sure when pseudo cysts can show, but i would assum thats the real deal!.
As far as your dates go they dont have to match to the date to be correct. They are “close” enough. If there was a discrepancy of like 3 weeks then there is a problen but a couple of days is fine. Its so early that you got lucky to see a sac. Yolk sac and baby wont be seen until hcg reaches 7,000 and 11,000 respectively.
If your hcg is way less than 1000 and you see that sac then something isn’t right. Long as your hcg is over 1000 at time of ultrasound scan i wouldnt worry.

Unfortunately my hcg was only in the 80’s at the time of the ultrasound… So, not looking good! I guess time will tell whether my levels double or not.

I went for a scan also at 4.6 weeks and was told they couldn’t see anything so for now it’s an unknown pregnancy, my hcg doubled from 200 to 489, I’m due again tomorrow for bloods. I had an ectopic last year and had bleeding for 9 days but now it’s only dark brown and streaky don’t know what to expect.
I’m no doctor but dark brown blood is usually old blood, and with those levels I wouldn’t really expect to see anything on the ultrasound. So by all means, it could still be a viable pregnancy. Good luck!

I’m not sure because when I first had bleeding it was red fresh blood, clots and had pinky watery blood too sorry for being too explicit and now it’s old blood.
I just pray and hope its a pregnancy in the right place and I have a healthy baby
Could be an endometrial cyst does have to be a pseudosac
Sorry I got it wrong my hcg levels were at 819 on Monday and today have gone up to 1297, but I’m still in the same boat as before it’s an unknown location of pregnancy. Due for another blood test on Friday and then a scan next week so just hope they see something this time in the scan

After an ectopic pregnancy, finding we are pregnant again can be a mix of emotions and I can completely understand your feeling nervous. It is perfectly normal to feel scared after the ordeal you have endured.
As you have mentioned that your scan showed a small gestation sac without a yolk sac or embryo, this is known as a Pregnancy of Unknown Location or PUL. This is not a diagnosis but a label until the final location can be identified with certainty. I cannot be sure, but it sounds likely that it may be too early to see a pregnancy on the scan.
I know you state you are sure of your dates but 48 hours either way would be acceptable.
If scans are inconclusive, doctors would carry out a series of blood tests to check hCG levels to see if the numbers are rising as expected. However, in isolation, hCG tests only provide a part of the picture - declining levels indicate that a pregnancy has ended and will be miscarried. It can also possibly indicate an ectopic pregnancy that is “self-resolving”. Also hCG levels rising by less than 66% over 48 hours means it is likely (but not certain) that it is ectopic. If levels rise normally, they suggest the pregnancy is implanted in the uterus. You may see from this that hCG levels only indicate certain possibilities and are not in themselves a definitive guide. A positive identification of an implanted embryo on a scan is usually needed before a final diagnosis/certainty that the pregnancy has implanted in the uterus. This is possibly the reason why your doctors are waiting until your appointment to carry out scans and tests.

Update on me… I had another scan on Friday and no pregnancy was found with hcg at 2999 doc decided to do a diagnostic laparoscopy. They said my tubes and ovaries looked fine but no pregnancy was found. Due for a scan on Friday hoping and praying they find it in my uterus. Now I’m back home coping with painful stitches, had a small cut near my belly button and on my bikini line.
Anyone else been through this could do with some advice on healing process please
Is your hcg doubling normally or are you having any pain? I’m surprised they jumped to surgery so quickly.
I had a laparoscopic surgery with my last ectopic, 5 months later and you can barely tell I had surgery- it does get better! Take it one day at a time and go easy on yourself.
Yes I was very surprised myself, one minute they were talking about medicine management and next it was surgery.
Yes they had doubled but I wasn’t in any pain. But I’m having more bleeding today. Could it possible be a missed misscariage? I was just speaking to a friend and she mentioned it and it has made me think!! Could your hcg levels still double if that was the case?

Maybe they were avoiding MTX in case there happens to be a viable pregnancy? Are you over 5/6 weeks? I know hcg numbers are important in what you can see on an u/s but I’d imagine dates are just as important.
I’d say probably not a missed miscarriage, I wouldn’t think that your hcg would continue to rise if it were.
And yes I’m lucky to have a 2yo little boy :), we ended up with an ectopic trying for a sibling. It’s comforting to know that I have had a successful pregnancy but that ectopic really ruined early pregnancy for me. I hope maybe your dates are off and it’s too early to see anything!
Yes they didn’t want to take any risks just in case it is viable.

Update, woke up this morning feeling a gush went to bathroom and I’m 99% sure I’ve misscarried, I passed out a plum size length clot and 2 other peices and now I’m having a heavy bleed. Was feeling dizzy and sick all day yesterday. Rang epu and she said they will still scan me on Friday and may be do my bloods again.
I’m having very mix feelings… I’m sad I’ve lost it as I now had some hope that all could be fine and then on the otherhand I’m thinking it wasn’t another ectopic.
Oh no I’m so sorry to hear! I hope they do some bloodwork before Friday so you can maybe have some answers during the wait. That if awful! Please go easy on yourself.
I’ve got a scan coming on Friday as well… I went to my OB last Friday and they were worried about some fluid on u/s near my right ovary. They had me go to the ER and were discussing termination even though I was barely 4 weeks, wasn’t in pain, and my numbers were more than doubling. I haven’t had any spotting but I’ve had weird ectopic-like pain on my bad side so I don’t know what to think. I don’t remember having that this early with my first. I’ve had three scans in the past 1.5 weeks and they’ve all suggested different things.
