HCG 3791 3 and a half weeks after MC.

My situation is so confusing, everybody’s puzzled by it, even medical professionals and I was hoping somebody might give me just a little insight if they’ve been through this before.

We found out we was pregnant on the 24th September, tests started to get fainter so I counted myself out and waited for the bleed. Big bleed (and I mean AWFUL, excessive blood, clots the size of plums and some serious cramping) happened on the 30th September and lasted about 6/7 days. I just got on with my life and that was that. I would of been about 5 weeks ish when the bleeding started.

On the 10th of October I decided to test, I thought with only being 5 weeks all the HCG would be completely out my system and was looking forward to drawing a line under it all. Well… the test came back super positive, as dark as control and digital was reading 2-3. I decided to book a private scan just to rule out anything nasty, like ectopic. On the 11th oct I went, they didn’t find anything resembling a pregnancy in tubes/ovary’s only a incredibly tiny fluid filled empty sac in uterus messing 2.9mm.

Now, I actually refused a referral to the EPU as I really wanted to give my body some more time and I knew what they would say, I know they’re policies (perks of working in the hospital trust, I looked them up) they would of told me to test 3 weeks after MC as the HPT can stay pos for a while after and if still positive after then, then they will investigate. I also refused the referral on another note because I’ve been through this before, I miscarried in July and the hospital tests came back negative, I was told all my tests (All 15 of them) were false positives and that what I’m experiencing was a heavy period, that was devastating and completely threw my whole life upside down emotionally, and I was scared to hear this again, especially after trying for a baby with my husband for 2 years. Stupid of me I now know.

I decided that if my pregnancy tests were positive after 3 weeks I will seek help. I kept a close eye on my body and became obsessed with tests because well you know, miracles do happen. All the tests between the 10th and now were all positive and I even got a 3+ on a digital so I knew that HCG was rising, albeit very slowly.

So yesterday (Monday) I decided to seek help, I had bloods, scans and a pregnancy test. Test came back roaring positive, scan showed nothing, anywhere! And HCG came back at nearly 3791. I was meant to go in on Wednesday for a repeat blood draw but had a phonecall after I got home from the hospital that they’re concerned with those levels and to come back in Tuesday for repeat scan with a consultant.

I’m soooooo confused and scared right now, how can my levels be so high and keep rising with nothing absolutely anywhere showing on the scan? Can this still be ectopic?? Everybody just keeps saying how bizarre it is, even medical professionals, it’s such a scary time.

Any insight would be great.

I am afraid I won’t be able to give you any clarity on what is happening as I am not medically trained in this field. I know how utterly worrying and exhausting it can be not knowing what is going on with our bodies and I hope you manage to get some answers today.

Hi karen, thank you so much for your replies, they help knowing they’re from somebody who has experienced what we have been through. I’m sorry it also happened to you.

I got discharged from hospital today, after being on nil by mouth for 24 hours! they decided to go with methotrexate instead as where the pregnancy is it could have complications with my uterus etc during surgery and my levels had gone from 3700 to 3079 in 48 hours. I ended up having 4x20mg injections into my side/buttocks area as they didn’t have a single 80mg injection! Ouch!

Now I’ve read a lot on here about people having the drug when they’re numbers are much lower than mine and a single dose didn’t work! I’m so anxious that this is not going to work on such high HCG? Have you heard of it working when HCG is high like mine? I’m just so anxious I’ll need the surgery now :frowning: I’m obviously keeping a very close eye on my body, I’ve been bleeding for about 4 weeks now, one big bleed at 5w pregnant and then spotting constantly pink with occasional fresh blood every now and then but now it is fresh blood constantly with some mild cramping that paracetamol does help. I would be 9W today.

Is it normal to have bleeding?

Also would today be day 1, the day the injection was given?

I understand what an anxious time this must be for you and it is also really worrying when we do not know exactly what is going on with our bodies.

to reassure you, Methotrexate is most effective in early ectopic pregnancy, usually when the pregnancy hormone ‘beta hCG’ level is below 5000 mIU/mL. Your levels were below this and are already dropping which is a good sign. We often see an increase in the day 4 bloods as the action of methotrexate is not instantaneous. Your doctors are looking to see a drop in your hCG value of at least 15% between days four and seven. If there has not been a 15% drop, this is when the doctors will consider a second dose of methotrexate or surgery. Day 4 is usually 4 days after the injection.

It is usual to begin to bleed as our hCG levels decrease and this bleeding can last between a few days and up to 6 weeks. This is not classed as your first period as it is a result of reducing hCG.

You can continue with paracetamol per pack instructions for pain, but please do not take any ibuprofen whilst your levels are reducing.

Day 4 today and they reduced from 3079 to 3009 which is a bit of a kick in the teeth! I was hopeful because they had dropped a big amount before even having the methotrexate I would of thought we would have seen a bigger drop today. Oh well, let’s hope it’s better news for day 7. After trying for a baby for over two years, this is a killer x

I’m sorry to hear there wasn’t a big drop but as it is common for hCG levels to rise on day 4 bloods, I would take a drop as a positive.

The problem with methotrexate is that it can feel like a long drawn out process. There is no way of telling how quickly our levels will call as it depends upon how high the levels were sho start with and our individual make up. What I will say is that it isn’t as invasive as surgery so try not to loose hope through these long days.
