Folic acid after mtx

I got a phone call last Sunday to say my hcg was down from 1200 on day of methotrexate injection to 5.3 (3.5 weeks after my injection) the doctor who rang me said that my hcg was now low enough and I didn’t need to come back for more bloods. She reminded me I can’t ttc for 3 months and said not to restart folic acid for 3 months ago. I was sure other medical professionals along the way told me to restart folic acid as soon as my levels were low enough so I could build it back up for when I do ttc again in 3 months. I asked her was she sure and she said she was. Now I’m confused as to when I can restart my folic acid and other prenatal supplements. Can anyone help me out?

I am so sorry to hear of your ectopic pregnancy loss,

It is advised not to take folic acid whilst the hCG levels are reducing as it can stop methotrexate from working. As soon as you reach pre pregnancy levels however, you are safe to start taking supplements again.

I do not know if your doctor has told you this as an individual case or if this is standard information she gives out, so I would check to clarify that there is nothing specific about your case that would warrant the wait.

If you have had either one or two injections of methotrexate, you should wait until your hCG levels have fallen to below 5mIU/mL (your doctor will advise you when this is through blood or urinary tests) and then take a folic acid supplement for 12 weeks before you try to conceive.
