Cornual ectopic Methotrexate 21 days HCG 384

Hi all am rob…This is with regards to my GF(32years) … who accidentally got pregant and we both were clueless until one day she complained of heavy abnormal​ bleeding.

  1. Apr 5th she was diagnosed cornual ectopic Left with HCG levels 4300 Sac size 3.6 cm. 7week 5 days pregancy

  2. We rushed to better hospital different city and Gyno and on April 8th her HCG levels were 2515 so she was given Methotrexate 50mg intra muscular,sayng that if it fails they have to undergo surgery.

  3. Doctor explained risk of rupture & hence she was admitted for a week further 4th day her HCG was 1484 and 7th day(7th April) 1184 with little bleeding. TVS on 7th (Apr 14th) Gestation Sac 3.66cm. doctor said it’s fine.

  4. Again after 7days (April 20th) her HCG levels were 484 next count April 27th hcg decreased to 321. Her next count is 3rdMay followed by TVS. Fingers crossed

  5. She is still complaining of a little pricking towards left at times and today she said even right portion is paining a little. But the pain is not permanent it just comes and goes. Side effects include cough.

  6. Going through forums pls help with experience are her HCG levels decreasing well & any chance of Tubal rupture still? Cos she is handling it alone iam very worried.

  7. How much more time for her HCG levels to come to under 5… it’s a difficult time I have been seeing her in pain … if it takes more she may lose her job.:frowning: Pls help and also pray we will be grateful to all of u always… regards…

I am so sorry to hear of your and your girlfriend’s ectopic pregnancy and loss.

It can be a very difficult and confusing time. When we experience ectopic pregnancy we are suddenly faced with a life threatening emergency and it’s treatment, reduction in fertility, concerns about the future and the loss of our babies. Experiencing any one of these is an ordeal, putting them together is immense and both your feelings are completely normal.

Experiencing some pain after an ectopic pregnancy is normal as the body has been through a great deal over a short period of time and abdominal pain is a normal side effect of the treatment. Many women experience pain due to adhesions (scar tissue that binds two parts of the body together) which form and take some time to settle. Her body may be preparing to ovulate and many women experience ovulation pain when they did not before their ectopic pregnancy - including myself. She also may have a heightened perception because of the sad loss that she has had to go through.

If she experiences worsening bleeding abdominal pain, shoulder tip pain or feels dizzy or unwell, she should seek urgent medical advice.

I am afraid if is difficult to predict how long it will take for your girlfriend’s hormone levels reach non-pregnant levels. As we are all individual, our bodies respond differently to the drug and it depends on factors like how high levels reached and our unique physiology. What I can say is that it can take a number of weeks and, while it can take some time, it is not as invasive a procedure as surgery. It is also encouraging that her levels are falling steadily although I appreciate that it can be a drawn out process. Also, in terms of chances of future successful pregnancies, studies do not show a significant difference between treatment routes, whether surgical, medical using methotrexate or expectant management (allowing time for the body to resolve the pregnancy itself).

This is with reference to my gf’s result for today’s tests.

Beta HCG level came down from HCG 321 to HCG 178 & TVS says size has slightly decreased. She was given shot on 8thApril today it’s been 25 days her levels have decreased from 2515 to 178.

She says her pain is also very occasional & back pain and other issues also gone.

But still we are worried as she is in flying job and might have to give up her career ,cos the medicine will take long and pain & tiredness will remain.

An idea if someone can comment , are the levels fine & can she travel back to her city or needs to be under supervision and any risk of Rupture still. Any cases before. As it’s 12th week that the pregancy is still there(3.6cm) so after 4 week of MTX can cornual ectopic still rupture.

Pls suggest natural ways to decrease the HCG level and precautions please.

Pls Comment , this forum has really been a stress buster for us and big support.

Dear Rob,

I am glad to hear your girlfriend’s symptoms are settling and the hCG levels are decreasing.

Regarding returning to work, women treated with Methotrexate sometimes choose to work through their treatment but many find managing the loss of a baby in this way too difficult to work through and need some emotional recovery time after treatment. Being treated with Methotrexate is also very tiring and work can therefore be exhausting.

Sadly, there is still the chance of rupture with low or declining hCG levels and until levels are at pre pregnancy levels, she does need to take things easy, no heavy lifting or housework. As she has a flying job, I would also be cautious incase something happens when she is in the air or she is too far away from her medical team. Unfortunately I am not medically trained so cannot give you advice on this matter. I would strongly urge her to seek the advice of her Dr or her occupational health department at work before returning to work.

The important thing is that she gentle with yourself. Although she may have made a good physical recovery, she may not be ready emotionally and should never feel embarrassed to approach her Dr and ask for a ‘sick note’ if she needs more time of work to emotionally recover. This is very common and doctors are supportive and very sympathetic to how women are feeling and are likely to write a note without any hesitation.

I want to first off say I am so so sorry to hear of your ectopic. I had my methotrexate shot on 7th April, so it seems like we started this rough journey at a similar time. My levels were lower (posted below for comparison), and yesterday, dropped to 30.

I am still in mild to moderate pain most of the time (mine was a left tube ectopic), with twangs and twinges on my left, and period like pains and aches in the centre, and some odd feelings and pains I cannot explain or describe around the general area!! I’m also still exhausted and struggling to think straight, which will again be down to the toll the drug takes on the body. I think this is all normal, unfortunately. :frowning: Sadly, it seems that although the lower your hcg levels, the lower your levels of rupture, it is still possible right until you are officially not pregnant (sub 5 hcg), so definitely make sure your partner takes it easy and looks after herself.

I think the general thing is, that this is a horrendous physical experience to endure; your partner’s body is doing an incredibly unnatural thing, and the methotrexate is a pretty nasty drug on top of that. It seems like one of those things that just takes time to get over and takes a lot of patience and TLC to get through.

I wish you both so much luck and hope your partner is able to finish her treatment soon without any complications.

My GF is fine and tomorrow is her HCG test… today she passed a brown colour discharge. Hope it’s ok.

Her sac size remains same.

I think still few more weeks to go…

Since last night she is experiencing little brown discharge and back pain since morning . Is it fine to have this kind of thing. Her level I below 100 now. .

Can an one guide how much more week to wait. It’s really testing our time.

Thanks for your email. My levels on day 37 (Sun 14 May) were 6… :frowning: So almost there, as it has to be 5 or less, but not close enough for them to legally discharge me. I have to go back again, just when I thought it was all over, on Day 41/42, but hopefully then it will be.

For reference, I ovulated on day 33. And my first period started on day 36. So I hope that your partner will be there soon too and her body will be trying to restart its normal cycles.

Thanks for replying so early. It feels good to be finding maximum information & data about the same thing what we are giving through. Congratulations for hitting HCG level6. Iam sure by now u r less than 5. Also do u still feel any kind of pain or prick if u walk or tend to exhert yourself.

My partner’s 33rd day(11th May) HCG was 88(decreased from 178level last week).

Let’s hope. Only problem is that her company is calling her back to work next week. Let’s see if the levels go down,

U said u ovulated on day 33, how can u differentiate that it was ovulation & not blood due to the ectopic. Am asking this cos the day my partner hit below 100hcg level she had a blood flow with pain towards left and back pain for 3days,now she has no bleeding but complains of pain / pricking towards the left side. She gets afraid tries to call & msg the Gyno,but the doctor is just not replying confidently. He says that it’s ok blood might come pain might be there and is not letting her discuss.

Just want to know are u going through any kind of such pain & what does your doctor suggests or guides.?

Anyways let’s see ,she is traveling back home and reaching in few hours. She is fine &, let’s hope for the best.

Day33rd she also started bleeding. I feel her body is understanding that the pregancy is going away so that’s why the discharge.

Usual time for beta HCG below 5 level is 42days. Let’s see

Praying that things go uneventful. Also worried about how much till the sac will take to resolve after levels come to normal.

My body feels pretty beat up, so yes, I still feel tired very quickly (I have a folate deficiency right now, I think this is the main reason), and I am still in pain (but it is period pain, and only a little other pain around my left side).

With regards ovulation/period, it is hard to explain to a man, but as a woman, you learn to differentiate between different pains, discharges and bleeds. Please forgive if this is ‘too much information’ but you asked, and I shall answer honestly about what I can to try and help you. :slight_smile:

Ovulation pain feels similar to what a rupture can (so a little scary!!) - a pulled muscle/stitch sensation, but I did not have any extreme/sharp pain (this would have been the warning sign of a rupture) or lots of blood (again, warning sign), or shoulder tip pain (again, warning sign).

In the few days before, my normal discharge returned (milky), then turned clear (the day of the ovulation sensations), then the day after, the stretchy, clear egg white mucus which indicated fertile time. The day after that, I had pink discharge (slight blood presence) which also supported my theory of ovulation. So that’s why I suspected that. If your partner is in tune with her body, she can also read these signs to perhaps try to identify what part of her cycle her body is in.

With regards the bleeding, typically the ectopic bleeding for me was either bright red (fresh) or very brown (old), and not in great quantities. Period blood is different (dark, red, different consistency) - I imagine your partner will know her own blood is different from the ectopic bleeding too. My period arrived with a LOT of pain, which I have need maximum dose of paracetamol and codeine for a day to manage (and even then, it was still extremely painful). But that pain I recognised as my period cramps, so I was not concerned (especially at hcg level 6).

I would say to expect discomfort and pains in general, and a little bleeding is normal, but be on guard for those warning signs (plenty more information on the Ectopic Trust site on those). The doctors really aren’t too helpful in being definite in their answers - they can’t see what’s happening inside and it’s different for every woman.

Hi all…

Today was the day 42nd for my partner .Last week she went through some blood , pain & stress we were expecting that levels will drop magically low.:)) But still it’s going down consistently at 50% decrease every week.

Her beta HCG for today day 42 after mtx is 40.(previous being 88)

Though we expected around 20s but 40 still decent number. Pain has reduced, tomorrow we r going for TVS to see how’s the size/shape of the sac has changed.

She is still worried , she is from the gulf airlines & the company is calling her back next week. We thought her levels will go down & she can plan by next week to go back. We were even positive that the sac size decreases​ & beta HCG goes normal & she can start flying as well.

But let’s see what God has in hands.

On the same day last week, Thursday, (day 41 for me though), I got the all clear - level of 3. It’s time to start rebuilding our life at last, ,thank goodness… The end WILL come for you two soon as well. I hope very much your partner’s employer is sympathetic and allows her the time she needs for her health…

Hey espoir, my heartiest congratulations to you for hitting the numbers at last.

  1. Regarding my partner on 18th may her TVS reports show the sac size decreasing by 5mm. Pain is also reduced.

  2. 23rd may , today her beta HCG is 19. It’s good we understood that her levels will fall by 50% to 55% every week. So hopefully next 10 days of wait for her.

  3. She is planning to go to back & travel back and resign from job & come back. We feel she still needs to rest for 1 month at least. Her sac size needs to be monitored as its curnual Ectopic and size increased from 3.6cm to 4.2 cm during medical treatment.

  4. We are hopeful nothing will happen, we choose to bear the financial problem that will come but won’t let the relationship suffer.:slight_smile:

  5. Got her levels checked today day 46 today. Reports says 19 hcg. So we r waiting

Thanks for your email and sorry for late reply. I ended up booking a last minute week’s holiday to go some place sunny and recuperate. I feel much better - full of energy (I have started taking supplements again, I drunk a lot of fresh orange juice and sat in the sun a lot) and mentally good again. (You will understand how hard physically/mentally this is… all I can say is I was broken and now I feel whole again :slight_smile: )

I’m so sorry I can’t answer your question about the size of sac - mine was a tubal ectopic. I was never told the sac size, only that after methotrexate it started moving into my body towards the ovary, and they didn’t know where it was going - it should have been reabsorbed by my body they told me. It didn’t come out. I am in no more pain, I just have niggles here and there which I think is caused by greater sensitivity to that region of my body.

I really really hope when you reply, you’ve reached that magic under 5, and your partner is feeling better. :slight_smile:
