When did you ovulate/have a period after methotrexate?

I’m on day 33 after methotrexate and my hcg levels on day 30 (this Sunday just gone) were 30… so I’m almost there!

Today, I’m 99.999999% I ovulated, even though my levels may/may not be under 5.

Weirdly, it was from the ovary that just gave me the ectopic pregnancy too… that one doesn’t know when to give up it seems!!! Crazy how I felt ovulation. It was so painful I thought I was rupturing… have never even noticed it before!

When did you ovulate/have a period in relation to your levels being 0/under 5 after methotrexate?

I have no idea what happens now, having been so focused on getting through the treatment. It would be really helpful to hear what happened to other women after they hit the magic “under 5” level.

Thank you.

Dear espoir,

I am so sorry to hear of your ectopic pregnancy and loss,

I too was treated with methotraxate but seven years ago so forgive me but my memory is a little hazy on timings of ovulation and periods but I can provide you with general information.

Many women experience pain when preparing to ovulate and many women experience ovulation pain when they did not before their ectopic pregnancy - including myself. You also may have a heightened perception because of the sad loss that you have had to go through.

Your periods can take a while to re-establish and they can re-start once hCG levels have fallen below about 100mIU/mL and most women find that their period arrives some time in the four weeks after their hCG levels have fallen to 0 if treated with Methotrexate.

The first period may be more painful or less so than usual, heavier or lighter, last for longer or shorter than usual – there really is no set pattern. You should be able to manage the discomfort with over-the-counter pain relief and should not be soaking a pad in less than an hour. If this is not the case, you should seek medical attention.

As a gentle reminder, with Methotrexate, you should wait until your hCG levels have fallen to below 5mIU/mL (your doctor will advise you when this is through blood tests) and then take a folic acid supplement for 12 weeks before you try to conceive. This is because the Methotrexate may have reduced the level of folate in your body which is needed to ensure a baby develops healthily. The Methotrexate is metabolised quickly but it can affect the quality of your cells, including those of your eggs and the quality of your blood for up to three months after it has been given. The medicine can also affect the way your liver works and so you need to give your body time to recover properly before a new pregnancy is considered. A shortage of folate could result in a greater chance of a baby having a neural tube defect such as hare lip, cleft palate, or even spina bifida or other NT defects. This is why the “wait” and then taking folic acid for 12 weeks before trying to conceive is so important.

Sending much love,

Karen x

Thank you so much for the reply.

I had my levels measured again this Sunday 14 May (day 37) … annoyingly they were 6. So I cannot be discharged. :frowning: So close, and SO far. I was so very sure I was over and done with hospital, but one last kick in the teeth. That being said, almost there, and can now look forward to recovery!

My doctor confirmed it was probably ovulation last Tuesday. And on Saturday I started my period… so what a whirlwind week. I think it’s going to take some time for my body to figure out which way is up, but first period is a good sign - I am hoping normality will kick in now!

I have to go back again for another blood test to get my magic under 5 result and be discharged - hoping to go on Thursday (day 41) and I can close this chapter in life…
