I received one dose of the methotrexate injection and my bhcg level is now below 5. It has now been a month since the injection, but I still feel unbelievably tired and started getting dark brown spotting again the last couple of days. Did anyone else also experience this? And did anything help for the fatigue?

I am so sorry to hear of your ectopic pregnancy loss.
It is quite common to experience tiredness with methotrexate injection. This certainly can last for a few weeks and don’t forget you have been through an awful lot lately too, which may be adding to your fatigue. If the tiredness continues or worsens, i would advise speaking to your medical team, it could be that you have another condition such as anaemia.
The current spotting could still be from your hormone levels decreasing. Some women and people can bleed for around 6 weeks after treatment. It is possible that it is the start of a period, however it does seem a little early. According to a study for women who had methotrexate treatment for an ectopic pregnancy in a Fallopian tube, periods generally return to normal after an average of 24 days (range of 24–38 days) from the pregnancy ending.
Again, i would monitor and seek medical advice if you have any concerns.
