HCG levels up on day 7 of Methotrexate

Hi all. New here, first eptopic. So many questions.

Was told on 11/13 I was having an ectopic pregnancy hcg was 1955. Was given methotrexate. HCG was 2355 on day 4 and now at my day 7 is 2838. I was told to expect it to be elevated slightly on day 4 but should see decrease by 15% which it’s actually up by 15%. I started bleeding today and have mild cramps as a period. Has anyone else experienced this?

Dear Lis,
I am so sorry about your ectopic pregnancy loss.
You are correct, we do expect a reduction in hCG between day 4 and 7. If there is an increase, some people go on to have a second injection. I would like to hope someone has discussed options with you.
As your hCG levels have increased and you have developed bleeding and pain, i would suggest seeking urgent medical advice.
Your early pregnancy unit should have given you details in who to contact with any worsening symptoms. If they haven’t and you are in the UK, i would suggest attending your local A&E unit, telling them you have been diagnosed with an ectopic pregnancy, your hCG levels have risen and you are now experiencing bleeding and pain. Although I do not wish to alarm you, these symptoms could be a sign of rupture, so it is always best to seek urgent medical advice to be on the safe side.

Sending much love,
Karen x
