Decidual Cast: Expectant Management

I was diagnosed with an ectopic pregnancy in my left tube last Thursday when I should have been 6 weeks +6. Up until that point I’d been bleeding with increasing frequency for about 7-8 days, but otherwise I’ve had no other symptoms (i.e. Pain). This along with my Hcg being quite low (210) and the mass being fairly small (20mm), I was told I was a good candidate for expectant management.

The issue I’ve found is that I haven’t had much guidance on what to expect with this, but at my 48 hour bloods on Saturday my hcg had risen slightly to 240.

I’ve continued to bleed pretty consistently the last few days, however today I passed what I think was a decidual cast. I’ve got another set of bloods tomorrow, but is it possible that passing the cast is a ‘good’ sign that things are progressing in the right direction?

I really want to avoid medical management/surgery if at all possible and it worried me that my hcg had gone up instead of down (although the fact that I had pregnancy symptoms at week 5 that abruptly stopped suggests that my hcg was previously a lot higher), but I’m really struggling with the uncertainty.

I am so sorry to hear of your ectopic pregnancy and loss,

Sadly it is difficult to tell what is happening to hCG levels inbetween blood tests. Bleeding is not necessarily a sign it is going down quickly so I wouldn’t want to get your hopes up too much. Our hCG levels all fall at different rates. I know the uncertainty is difficult but hopefully they are heading in the right direction.

I presume you will be having another blood soon to check what your levels are doing. I hope they are falling steadily.
