I’m 39 and have no children. My first pregnancy devastatingly ended in a MMC at almost 10 weeks and I had surgical management on the 14th November.
On the 13th January, I had a very faint positive pregnancy test, followed by a few more. On the 16th my HcG was just 54 and two days later it only rose to 63. 72 hours later it was at 164 and a week after was at 444. That was Monday morning. An internal scan showed thickened uterus walls but no sign of a sac anywhere. There were no masses on my ovaries and nothing visible in my tubes. There was no free fluid either. Yesterday my HcG had risen slowly again - to 580. We saw a senior consultant who explained this was a PUL being managed as a suspected ectopic. I’m back tomorrow for another HCG test and again on Monday for another HCG test and scan.
I’ve decided against using methotrexate because I really don’t want the drug in my body and I’ve read too many stories of a second dose being needed and even surgery following that. The literature says it puts TTC on hold for theee months but my consultant explained that it’s actually more likely to be six month. Surgery means risking my tube.
Has anyone been through this - low and slow rising HCG, PUL etc. I’m concerned that they can’t locate the pregnancy but the levels are slowly going up. Any comments or experiences welcome as I’m breaking my heart right now and feel so unsure. X

I had expectant management with PUL which later got diagnosed as ectopic. My hcg would go up 100 Down 90 etc. Began bleeding around 26th October and was eventually discharged 5th December. For the first 3 weeks it basically had stayed the same though changing rapidly between 48hr slots so each time I thought it was over.
Be kind to yourself, the regimen on 48hour testing is mentally exhausting especially if you get good or bad news.
Are you bleeding? What made them start the hcg testing? I had started bleeding which brought me in.
I believe if it goes over 1000 they start looking at Methotrexate if it doesn’t go down naturally. Take it easy - no heavy exercise. You will get through this.

I’m so sorry to hear you are going through this worrying time. I know how frustrating a PUL can be and how it brings up so many emotions. It’s reassuring that you are being closely monitored. I’m not medically qualified to advise on the best way forward. If the HCG continues to rise, the doctors will make an assessment. An ectopic pregnancy can not remain in place. It may be managed, but it may need medical intervention.
Over these next days, look after yourself. If you have any increasing symptoms including abdominal pain, shoulder tip pain, or bowel problems, it is important to be seen right away at your A&E. (https://www.ectopic.org.uk/patients/sym … diagnosis/) NHS 111 line is also available to help triage any symptoms you are having. I do not mean to worry you, but with rising HSG and a PUL, there remains a risk of rupture.
